Anyone can report incidents of discrimination, harassment or bias using an online reporting form. Students will receive confirmation that their report has been received, and a staff member from the Office of Compliance (OC) will follow up to discuss next steps. Students may alternatively be contacted by the Office of Student Assistance and Support (OSAS). Online Reporting Form
Employees can learn about reporting and response options regarding the Hostile and Intimidating Behavior Policy on the human resources HIB website. Employees can request a formal investigation of HIB from the Office of Compliance using the same Online Reporting Form above.
Student Support Resources
- Office of Student Assistance and Support (OSAS) offers guidance and support for all students dealing with personal problems, including reporting an incident, academic resources, and helping someone you know. OSAS will help with addressing problems and finding who to contact. Schedule an appointment here.
- University Health Services (UHS) provides confidential, no-cost mental health services. UHS also provides a 24-hr on-call crisis line at 608-265-5600, ext. 9. Schedule an appointment here.
Employee Support Resources:
- Employee Assistance Office (EAO) helps faculty and staff maintain and enhance their personal and professional lives and offers services to promote emotional well-being. This office will keep information as confidential as the law allows. Email to schedule an appointment. EAO’s services also include LifeMatters, with 24/7 access to professional counselors.
- The Ombuds Office facilitates understanding, communication, and resolution of conflict for employees. The office offers confidential and neutral help to people on campus so they can communicate more effectively with each other. A person from the Ombuds office will listen to your concerns, explain procedures, and talk about your choices. If you ask them, they may be able to serve as an intermediary as you try to resolve disputes. The Ombuds office can also give you confidential advice.
The Department of Kinesiology is developing an internal system to support students, faculty, and staff who experience incidents of bias, hate, and/or microaggressions. Once established, the information will be published on this webpage.
